Radio surgery cutting current can be more versatile than a laser for smoothing and finishing skin wounds or revising scars after skin cancer surgery. Radio frequency current technology that will help Physicians/ Surgeons to fine tune the repair of surgical defects is a new application that will help surgeons make the best use of their investments in the instrument.
      Patient satisfaction with this technique is very high because radio surgery causes less bleeding than conventional surgery, allowing the post operative skin defects to be more easily camouflaged.
Other advantages: Faster wound healing and ability to remove the sutures in 24 hours compared to scalpel incision that is on the fifth or sixth post operative day.
High Current: Higher intensities can be used for complete excision of skin lesions and debulking of scars.
Low Current: Radio frequency at a medium energy level produces uniform, shallow vaporization of tissue as the instrument is run tangentially across the skin. Use of the cutting current, instrument for fine corrections is enhanced by use of a surgical loupe with 2X to 4X magnification or a least with simple head loupe plain, assuring good visualization and smoothing of small surface irregularities with pin point accuracy.
DIFFERENT TYPES OF WIRE ELECTRODE AND USAGES:
Wire electrodes and loop electrodes available for different applications. Mostly wire electrodes are used for cutting & coagulation work, even bigger area to be sliced off. Otherwise wire loop electrode is used depending upon the size and area located. Power output should be set slowly from the lower level as per the surgeon’s need. However the required output can be selected by surgeons after doing a number of cases.
Electrode Surface Area: The larger the electrode, the greater the heat & the need to increase the power supply. Thinner the needle, the less heat will be produced & less power supply. Thinner the needle, the less heat will be produced & less power supply.
Wave form: Cutting wave form- least heat.
Haemo: Some lateral heat coagulation, more lateral heat frequency.
Frequency: ¾ Higher the frequency, less lateral heat produced.
Temperature Achievement:
A small electrode (0.5mm thick) is used for lower power setting and less lateral heat is produced. A thinner and finer electrode is mostly used in many places, which is better than using a thicker electrode resulting in need of higher power setting and no significant difference in tissue destruction. A variety of electrodes are available but the standard set of 7 electrodes can be used effectively. Needle electrodes for incisions: Loop electrodes excise small skin lesions or to shave superficial lesions. A diamond shape electrode is used for elliptical excisions. A very fine thinner needle (0.3mm thick) is for epilation work.
VARIOUS APPLICATIONS
Plastic Surgery: Face lift, Breast Reconstruction, excision of skin lesions. Blepharoplasty.
Treating Facial Telangiectasias:
      Removal of telangiectasias by conventional methods like Electro- cautery etc.: Patients experience discomfort and intolerable pain-efforts and surgery must be discontinued. In addition, some patients will develop small dells or depressions associated with dessication, crust formation and healing. 
     Dermatology-Warts: Warts are common cutaneous and many different means have been used in treatment of them. A Loop electrode ¼ diameter (6.35mm) is inserted around the wart and pushed down the center rather than straight across. Care must be taken not to penetrate into the dermis because this may lead to scar formation. Little scarring , if any, will result in the affected epidermis alone being removed and the dermis left intact. For very smaller warts and flat warts, loop (1-2mm in diameter) electrode is not advisable, and pointed electrode is commonly used. Loop electrode is suitable for periungual and subungual warts, condyloma accuminata and plantar warts.
 The frequency radio surgical instrument has proven to be a boon for dermatological and plastic surgery, as it allows many more procedures to be accomplished in an office setting. In addition, patient acceptance has been high, because of more rapid healing time and superior cosmetic results as with cold knife. For example, facial telangiectasias by radio frequency minimized discomfort and completely eliminated dells or depression and severe pain or bleeding. With it, one can treat the patient’s face and neck area in one sitting and it can also be operated in very low current setting compared to other methods with higher frequency.
Eyelid Surgery: Excellent results have been obtained removing seborrheic keratosis from the upper eyelid and there are many papers describing Blepharoplasty and similar papers.
E.N.T: Radio assisted Uvulopalatoplasty, removal of vascular lesion within the mouth, Epistaxis Hemangiomas and tongue lesion removal.
General Surgery: Simple incision/biopsy is very easy to perform as described earlier. Varicose veins, including thread veins, have been successfully treated.
Podiatry: The excision of plantar fibromata verrucae and ingrown toe nails have all been widely documented with good results. Many surgeons now use nail matrix phenolization for the management of ingrown toe nails. Instead of applying a phenol swab, an insulated nail electrode can be used and the germinal epithelium destroyed using the coagulation current. This gives faster immediate post operative healing and recurrence rate approx. 5%, and 95% cure rates reported.
Vasectomy: Several papers have described successful techniques for this operation using the cutting waveform to incise the scrotal skin and the coagulation waveform to destroy and seal the vas.
