Dosage: Each tablet typically contains 45 mg of dextromethorphan and 105 mg of bupropion.
Form: It is an extended-release (ER) oral tablet, which means it is designed to release the medication slowly into the body over time. It is crucial to swallow the tablets whole and not to crush, chew, or split them. Doing so could lead to a rapid release of the drugs and increase the risk of side effects.
How it Works:
The exact mechanism of action for this combination in treating depression is not fully understood, but it is known to affect several neurotransmitters in the brain.
Dextromethorphan: This component is known for its historical use as a cough suppressant. In this combination, it acts as an N-methyl D-aspartate (NMDA) receptor antagonist and a sigma-1 receptor agonist. These actions are thought to help improve depressive symptoms.
Bupropion: This is an antidepressant that works as a norepinephrine and dopamine reuptake inhibitor (NDRI). It is also used alone to treat depression, seasonal affective disorder (SAD), and for smoking cessation. In this combination, bupropion serves a dual purpose: it has its own antidepressant effects and it acts as a strong inhibitor of an enzyme (CYP2D6) that breaks down dextromethorphan. This inhibition allows the dextromethorphan to remain in the body longer and at higher levels, making it more effective.
Usage and Dosing:
Dextromethorphan and bupropion tablets are used to treat adults with MDD.
The medication is typically taken once daily for the first three days, and then the dose may be increased to twice daily, with doses taken at least 8 hours apart.
It can be taken with or without food.
Important Considerations and Side Effects:
Suicidal Thoughts and Behavior: Like other antidepressants, this medication has a boxed warning about the risk of suicidal thoughts and behaviors, especially in young adults (ages 18-24) during the first few months of treatment or following a dose change.
Common Side Effects: Dizziness, headache, diarrhea, dry mouth, excessive sweating, and sexual function problems are some of the common side effects.
Serious Side Effects: Rare but serious side effects can include seizures, increased blood pressure, manic episodes in people with bipolar disorder, and a potentially life-threatening condition called serotonin syndrome.
Interactions: It is very important to inform your doctor about all other medications you are taking, as this drug can interact with many others, including MAOIs, SSRIs, and other drugs that affect blood pressure or the central nervous system. Do not take this medication with any other products containing bupropion or dextromethorphan.
